Passports for applicants under 16 years of age

First time applicants under the age of 16 born outside of the U.S. must first establish their claim to U.S. nationality before a U.S. passport will be issued. Please refer to our information regarding how to apply for a Consular Report of Birth Abroad.

The application must be signed by the parents/legal guardian of the applicant in the presence of a Consular Officer at the at the Consulate General serving your area. This requirement cannot be waived. Services are provided by appointment only, which can be scheduled online.

Completing the application

Please type or print legibly when completing all sections of the application.

Print out and submit only one-sided pages of the application, and bring all required supporting documents in their original format.

The following must be presented in person at the Consulate:

  1. Completed (but not signed) passport application (Form DS-11).;
  2. Applicant (child)
  3. Child’s most recent U.S. passport. If the child has never held a U.S. passport, documentary evidence of U.S. citizenship must be provided;
  4. Child’s U.S. or foreign birth certificate, which identifies his/her parents
  5. U.S. regulations require the consent of both parents/legal guardians for the issuance of a U.S. passport to a minor child. If only one parent or legal guardian submits the application for a minor child, the absent parent or guardian is required to complete a notarized consent form (DS-3053) to express their consent for the issuance of the passport;
  6. Valid photo identification of the child’s parents/legal guardians
  7. Two (2) U.S. sized passport photographs. (Note: these are a different size from Australian passport photographs). Please refer to our list of photographers;
  8. Applicable fee (All fees are subject to change without notice)
  9. Self-addressed Express Post, Express Post Platinum or Registered Post envelope for return of the passport.

Helpful hints for applications made via the Sydney Consulate:

  • Documents submitted with an application must include the original document and one clear photocopy. The consular fee for photocopying is USD 1.00 per page;

  • Make a copy of all supporting documents (not the application forms) to leave with us. If photocopies are not provided, you will need to pay $1 USD for each photocopy (if you are also applying for a Social Security Number, please make two (2) copies of each supporting document);

Photo Guidelines

Two identical color photographs required 50mm × 50mm (2" × 2"); headsize chin to top of head minimum 25mm (1"), maximum 35mm (1 3/8").
