Passports for applicants under 16 years of age
First time applicants under the age of 16 born outside of the U.S. must first establish their claim to U.S. nationality before a U.S. passport will be issued. Please refer to our information regarding how to apply for a Consular Report of Birth Abroad.
The application must be signed by the parents/legal guardian of the applicant in the presence of a Consular Officer at the at the Consulate General serving your area. This requirement cannot be waived. Services are provided by appointment only, which can be scheduled online.
Please type or print legibly when completing all sections of the application.
Print out and submit only one-sided pages of the application, and bring all required supporting documents in their original format.

Two identical color photographs required 50mm × 50mm (2" × 2"); headsize chin to top of head minimum 25mm (1"), maximum 35mm (1 3/8").
