Additional visa pages

Additional visa pages will not be added to an expired, damaged or mutilated passport, or to passports that were issued with 1 year or less validity. No more than two sets of visa pages will be added to any one passport.

Completing the application

Please type or print legibly in black ink when completing all sections of this application.

Please disregard instructions on the online forms, and submit the following by mail to the Consulate General serving your area:

1. Fully completed and signed application form (DS-4085).
2. Current U.S. Passport
3. Self-addressed Express Post Platinum or Registered Post envelope for return of your passport.
4. Applicants under age 16: One parent is required to sign the application. Please submit the child’s original birth certificate and the signing parents original photographic identity such as a valid passport;

Do not submit your application directly to a U.S. based passport agency - it will be forwarded to the Consulate General serving your area for processing.

There are no fees for this application.
